3. Safety box could not be closed, windows didn't open, window blinds position could not be changed, toilet was not working right, lavatory was simply ridiculous (about 100 sqi surface and 3 feets height).

4. Shall I list a few reasons?:Broken elevator, dirty rooms, constantly flowing toilets, broken shower head, leaking tub, lumpy squishy mattresses, dark rooms, ill-fitting, in-window AC so street noise is ridiculous ALL night.
